I got this response from them:

The interface #4 "SimTech Wireless HS-USB Ethernet Adapter 9000" needs a 
special tool from Qualcomm for dialing, it's not open for us. We don't 
suggest you use it.

At least this confirm that the chip is from Qualcomm.
