Yet another try with VFIO on SPAPR (server PPC64).

After a previous try, the series was split into few smaller ones,
first one was "spapr_pci: Prepare for VFIO" which prepares spapr_pci.
This one is the second one and prepares vfio. Third one will be posted
after series 1 and 2 meet in some external tree.

Changelog is in individual patches.
