The upcoming virtio 1.0 standard requires virtio devices not to
rely on iov boundaries when parsing requests or sending responses.
This series converts virtio-scsi.

v1->v2: missed a few replacements in patch 6 (Paolo)
	never compare in_num/out_num to a non-zero value (Paolo)
	do not use in_sg[0]/out_sg[0] (mst)
