Preliminary to further ISA API cleanups...

Convert this sequence:

- portio_list_init()
- portio_list_set_flush_coalesced()
- portio_list_add()

to portio_list_register_flush_coalesced(),
and the non-coalescing equivalent:

- portio_list_init()
- portio_list_add()

to portio_list_register().
