[PATCH rfcv2 12/18] vfio: Initialize host IOMMU device and pass to vIOMMU

Initialize host IOMMU device in vfio and pass to vIOMMU, so that vIOMMU
could get hw IOMMU information.

Support both iommufd and legacy backend.
