Since we are going to re-use access to the
MachineClass::pci_allow_0_address field, factor the
current code as a new machine_refuses_bar_at_addr_0()
helper. In pci_bar_address() invert 'allow_0_address'
logic as 'bar_at_addr_0_refused'.
