When pasid is enabled, any hwpt attached to non-PASID or PASID should be
IOMMU_HWPT_ALLOC_PASID flagged, or else attachment fails.

Change vtd_destroy_old_fs_hwpt() to pass in 'VTDHostIOMMUDevice *' for
naming consistency.
