Public bug reported:

Applies to both qemu and qemu-kvm 1.0, but only when kernel is 64bit and
userspace is 32bit, on x86.  Did not happen with previous released
versions, such as 0.15.  Not all guests triggers this issue - so far,
only (32bit) windows 7 guest shows it, but does that quite reliable:
first boot of an old guest with new qemu (or qemu-kvm), windows finds a
new CPU and suggests rebooting - hit "Reboot" and in a few seconds it
will be locked up (including the monitor), with 100% CPU usage.
Killable with -9.

Actually after trying to do lots of experiments and finally a git
bisection, it turned out that the issue only affects qemu-kvm, not
upstream qemu.  Bisection between qemu-kvm 0.15.0 and 1.0 lead to this
commit:

Backtrace stopped: Not enough registers or memory available to unwind further

which is not entirely interesting, but:

when exiting gdb (I attached it to a running process), the whole thing
unfreezes and continue its work as usual, if no lockup ever occured --
ie, it is enough to attach gdb to a locked up process and quit gdb -
enough to unfreeze it.  Also, when running under gdb, the lockup does
not occur - I can reboot the guest at will any times, it all goes fine.
Once gdb is detached, reboot immediately results in a lockup again -
which - again - can be "cured" by attaching and detaching gdb to the
process.

And one more correction for the original report.  When locked up, it
does NOT use 100% CPU - CPU is 100% _idle_.

32-on-64 doesn't build on Fedora due to glib2-devel.i686 conflicting
with its x86_64 cousin, so I can't reproduce this.  Can you try
bisecting this further?

$ git bisect start M M^

$ git merge --no-commit M^
[build and test]
$ git merge --abort (or git checkout -f)
$ git bisect [good|bad]

if it happens that M^2 was the bad commit, you'll get a merge conflict. 
In that case do

$ git merge M

(you'll just be testing M again, but it's good to verify)

I tried bisecting it today, again.  I think you did mean
145e11e840500e04a4d0a624918bb17596be19e9 as the "M" point (the large
merge), not b195043003d90ea4027ea01cc7a6c974ac915108 (which is the first
commit in that merge) ;)  Actually I did the same already, just didn't
post to the bugreport.  But anyway.

The first bad commit inside the merge which shows the bad behavour is:

Note: the issue happens only with qcow2 being in use so far, directly or
indirectly with -snapshot.

Also note that it only happens within kvm tree, ie:

I tried to debug it further but without much success.

I'm attaching an strace the above source (with a few debug
fprintf(stderr)s added into qcow2 source around lock/unlock calls)
running winXP guest from point where I hit "Reboot" button and up to the
point where it stalls.  Search for "qcow2: mutex_unlock" from the _end_
of the file.

What I also observed is -- it looks like it merely loses an interrupt
somewere, it is enough to Ctrl+Z/fg it or to attach/detach strace to the
process for it to "unstuck" and continue executing.  Attaching gdb also
makes it unstuck as demonstrated initially.

This qcow2 commit may actually be innocent: it just started using
coroutines, and that's where the bug/problem might be, say, 64bit kernel
gets confused by the process switching stack for example?  I dunno.

Note again that switching to alternative coroutine implementations makes
the issue go away.  Ie, it only happens with mkcontext&Co coroutine
implementation, and the commit which git bisect found to be "guilty" is
the one which makes usage of coroutines.

After some more digging I found out that qemu also has this very issue,
but it happens a bit differently.  In particular, this very same winXP
test guest freezes in upstream qemu with -enable-kvm on _shutdown_, not
on restart.  In qemu-kvm it happens on restart but not on shutdown.

And bisecting plain qemu leads to this commit:

  qemu/kvm locks up when run 32bit userspace with 64bit kernel

Status in QEMU:
  New
Status in “qemu-kvm” package in Debian:
  Confirmed

Bug description:
  Only applies to qemu-kvm 1.0, and only when kernel is 64bit and
  userspace is 32bit, on x86.  Did not happen with previous released
  versions, such as 0.15.  Not all guests triggers this issue - so far,
  only (32bit) windows 7 guest shows it, but does that quite reliable:
  first boot of an old guest with new qemu-kvm, windows finds a new CPU
  and suggests rebooting - hit "Reboot" and in a few seconds it will be
  locked up (including the monitor), with no CPU usage whatsoever.
  Killable only with -9.

The bug in Debian has been marked as "Fix released", so I assume this
has been fixed in upstream QEMU, too? Or is there still anything left to
do here?

Since the debian bug has been marked as fixed and there wasn't any
response to the question in my last comment, I assume this has been
