Third round, primarily dropping IOAPIC relocation support:
- replace IOAPIC relocation with a qemu-kvm compat patch
- fix ioapic_eoi_broadcast name
Note that, even if we do not include the base address explicitly in the
IOAPIC state, we will still need to read it out for KVM in-kernel
support as its ABI considers the base address part of the IOAPIC state.
