Not much. First, renaming DID_NEXUS_FAILURE and BLK_STS_NEXUS to
DID_RESERVATION_CONFLICT and BLK_STS_RESERVATION respectively. Second,
adding a __ in front of those DID_* constants that are ultimately
changed back to DID_OK.
