The patch below does not apply to the 5.15-stable tree.
If someone wants it applied there, or to any other stable or longterm
tree, then please email the backport, including the original git commit

Most routines in qla_bsg.c call bsg_done() only for success cases.
However a few invoke it for failure case as well leading to a double
free. Validate before calling bsg_done().
