Changes in v4:
- rebased over master
- fixed a nasty bug in CTNFRQ setting that broke unrelated code
- fixed a bug in Tegra's psci_cpu_off that prevented powergating
- factored out armv7_get_cpu_id macro
- removed duplicate call to psci_arch_init
- addressed review comments regarding comments
- wait on initial CPU power-down using Flow Controller instead of PMC
