In this patch, some code is factored out of construct_vmcs() to
create vmx_set_host_vmcs_fields(). If we decide to create a separate
vmcs function for PVH again, both call call vmx_set_host_vmcs_fields.
Changes in V11:
- Rename to vmx_set_host_vmcs_fields.
