32-bit PVH-classic support

* Added patch 2 that prevents a 32-bit PVH guest from turning off PAE

* Add xenpmu_op hypercall to pvh_hypercall32_table[] (patch 3)
* Adjust 'is_pvh_domain(currd)' test to match a similar test further in the
routine (patch 3)

* Dropped a few unnecessary tests for 32b mode
