This wiki page - http://wiki.xensource.com/xenwiki/XenParavirtOps - says
that the kernel to use with 4.0.1 is 2.6.32.19:
"
pvops kernel git branches that require new IOAPIC setup hypercall from
Xen hypervisor 3.4.3 or 4.0.0 or newer to run as dom0:
* xen/stable-2.6.32.x - this is the long term maintained branch, based
on the kernel.org long term maintained Linux 2.6.32.x. This is the
default branch for Xen 4.0.1. Currently it's at 2.6.32.19. Changelog:
http://git.kernel.org/?p=linux/kernel/git/jeremy/xen.git;a=shortlog;h=xe
n/stable-2.6.32.x
"
When I look at the changelog URL it says that 2.6.32.18 is the latest,
and when I check it out using the instructions on the wiki page I also
get 2.6.32.18 not .19. On another server that I checked out sometime in
the last few weeks, it has 2.6.32.19 on branch stable-2.6.32.x. When I
now git pull on that server it tells me I'm ahead by ~110 commits.
Has something changed in the tree somewhere that has rolled back
stable-2.6.32.x?
James

2.6.32.19 contained some upstream fixes which broke the Xen toolstack so
xen.git was rolled back to 2.6.32.18. (specifically the new stack guard
page broke the toolstack's use of mlock.)
The 2.6.32.21-rc contains the fixes for that breakage so I guess we can
roll forwards again when that is released.
Ian.

Yeah, it was rolled back to 2.6.32.18. I just fixed the wiki page.
