 Changes in irq.c as PVH doesn't use vlapic emulation. In mtrr we add
 assert and set MTRR_TYPEs for PVH.

Changes in V2:
   - Some cleanup of redundant code.
   - time.c: Honor no rdtsc exiting for PVH by setting vtsc to 0 in time.c
