[PATCH RFC 00/55] x86: use domheap page for xen page tables

This series switches xen page tables from xenheap page to domheap page.

This is required so that when we implement xenheap on top of vmap there won't
be a loop.

It is done in roughly three steps:

1. Introduce a new set of APIs, implement the old APIs on top of the new ones.
   New APIs still use xenheap pages.
2. Switch each site which manipulate page tables to use the new APIs.
3. Switch new APIs to use domheap page.
