[1/5] x86: mpparse and cstate need to use 32bit apic id
[2/5] x86: increase MAX_LOCAL_APIC
[3/5] x86: x2apic preenabled but intr-remapping is not enabled
[4/5] x86: adjust x2apic section placement
[5/5] x86: adjust other interrupt related section placement

Some of these originate from Yinghai Lu.

Caveat for all of the x2apic code changes is that I'm not able to
actually test that I didn't broke anything from what I got from
Yinghai, lacking respective hardware.
