FYI, I am testing the hp portion and hit and existing oops in
radix_tree_gang_lookup (fixed by upstream block layer patch):

I saw the problem when doing this sequence:
1) multipath -F
2) "rmmod qla2xxx"
* This would trigger the oops

Subject: scsi_dh: Use SCSI device handler in dm-multipath

This patch converts dm-mpath to scsi hw handlers. It does
not add any new functionality and old behaviors and userspace
tools work as is except we use the safe clariion default instead
of using the userspace setting.
