[patch ia64] remove duplicate uint64_t definitions

Dan,

I needed the following patch to build xen-unstable.bk on ia64/SLES.  Good
work on getting this in bk!

Thanks.  Yes this was the result of a temporary workaround
of some code that got removed, then replaced in common
code.

Thanks, but kudos are also due to Keir and Ian who helped me
through a large number of changes to common code.
