I am trying to find out what support exists today for hardware IOMMUs in Xen. 
In particular, I am interested in how the IOMMU support is used in guest and driver domains.   

Any information (including pointers to the source code or documents) will be much appreciated.

AMD + Intel.

Google for "Intel VT-D specification". Take a look at

xen/xen/drivers/passthrough/* has lots of source code.
