Implement primitives necessary for the 4th level folding, add walks of p4d
level where appropriate, and remove __ARCH_USE_5LEVEL_HACK.

Implement primitives necessary for the 4th level folding, add walks of p4d
level where appropriate, and remove __ARCH_USE_5LEVEL_HACK.

Implement primitives necessary for the 4th level folding, add walks of p4d
level where appropriate, and remove __ARCH_USE_5LEVEL_HACK.
