The quilt patch titled
     Subject: docs-mm-add-document-for-swap-table-fix
has been removed from the -mm tree.  Its filename was
     docs-mm-add-document-for-swap-table-fix.patch

This patch was dropped because it was folded into docs-mm-add-document-for-swap-table.patch
