[merged mm-hotfixes-stable] selftest-fix-headers-in-fclogc.patch removed from -mm tree

The quilt patch titled
     Subject: selftest: fix headers in fclog.c
has been removed from the -mm tree.  Its filename was
     selftest-fix-headers-in-fclogc.patch

This patch was dropped because it was merged into the mm-hotfixes-stable branch
of git://git.kernel.org/pub/scm/linux/kernel/git/akpm/mm

fclog.c does not compile because it is missing fcntl.h, needed for
O_RDONLY etc.

There are also some redundant includes that are also in
kselftest_harness.h.
