summary: Clean up block-device hotplug routines in xenlinux.

* xend starts without problem
* NOT able to boot any HVM domains. The system gets a CPU PANIC as soon
as you start xm-test. This happens during the sanity check. I opened
defect #567.

* Bug# 562 is present
