Add a test for read and write IOPS throttling.

Changes compared to v1:
- Changed ". tests/block/rc" into ". tests/throtl/rc".
- Split test() into run_test() and test() to eliminate duplicate error handling
code.
- Made the test whether the iocost controller is supported more robust.
- Made the test easier to debug.

This check above is in group_requires(), so I dropped it.

I added set_conditions() here, so that this test can be run for both
null_blk and scsi_debug. It also makes this test case consistent with
other test cases in this group.
