Here we go again... Second or third time I lost my LVM... :(
What's wrong this time?

Which LVM (tools+driver) version?
What exactly happened before vgscan failed?

I had plugged in two new harddisks, but from what I can tell, ID's AFTER
the last pv disk/partition...

I did accidently boot it up with a new disk IN BETWEEN a PV, and that gave some
errors... I noticed this, and moved the two new disks to a higher ID...

Did anyone come to any conclusion about what could have happened to
my 'unknown VG'?
