[PATCH] scsi: Fix more error handling in SCSI_IOCTL_SEND_COMMAND

Fix an error path in SCSI_IOCTL_SEND_COMMAND that calls
blk_put_request(rq) on an invalid IS_ERR(rq) pointer.
