Use a flexible array member to combine allocations.
Add __counted_by for extra runtime analysis.
Change dwc3_qcom_clk_init to a single parameter. No need to pass count
separately now.

I don't see how this is better than before, especially when it's
deviating from the new dwc3-qcom logic. Please review the dwc3-qcom
instead before making changes to the legacy driver. I prefer new
development to be done in the new dwc3-qcom driver.
