if CONFIG_SWAP is not defined we get:

mm/vmscan.c: In function ‘remove_mapping’:
mm/vmscan.c:387: warning: unused variable ‘swap’

Convert defines in swap.h into blank inline functions to fix this warning
and be consistent.
