The changes are:

 - enhance sense data reporting
 - enhance ioaccel command support
 - add in block layer tag support
 - clean up resets
 - update copyright
 - add in new controller id

* changes from v4
 - changed byte extraction of resid_cnt to get_unaligned_le32 (Hannes Reinecke)
